ADORA2A

腺苷A2A受体(ADORA2A)是嘌呤能受体家族成员,又称A2AR,广泛分布于中枢神经系统、免疫细胞及心血管组织。ADORA2A通过与内源性腺苷结合,调控细胞内cAMP水平,影响多巴胺能神经传递,在运动协调、睡眠调节和免疫抑制中发挥关键作用。研究显示其活性异常与帕金森病的运动功能障碍密切相关,阻断该受体可增强左旋多巴疗效。在肿瘤微环境中,A2AR激活会抑制T细胞抗肿瘤活性,这一发现推动了其与PD-1抑制剂联用的临床试验。
目前已有选择性A2AR拮抗剂伊曲茶碱(istradefylline)在日本获批用于帕金森病治疗,可改善剂末现象。针对癌症免疫治疗领域,多个小分子抑制剂如AZD4635已进入II期试验,与免疫检查点药物联用显示出协同效应。在神经精神疾病方面,A2AR调控异常还与成瘾行为、双相情感障碍相关,相关药物研发正探索其对认知功能的调节作用。ADORA2A同时成为咖啡因的作用靶点之一,解释了咖啡因提神效果的部分分子基础。
