CFD

补体因子D(Complement Factor D,CFD)又称脂肪细胞特异性分泌因子Adipsin,是补体系统旁路途径的核心蛋白酶。CFD由脂肪细胞及肝脏合成,在血液中以低浓度持续存在,通过特异性切割补体因子B激活补体级联反应,形成C3转化酶放大炎症信号。CFD异常表达与年龄相关性黄斑变性密切相关,其过度激活会导致视网膜色素上皮细胞损伤,全球约15%的AMD患者存在CFD基因变异。在代谢性疾病领域,CFD水平升高可加剧胰岛素抵抗,与2型糖尿病肾病进展呈正相关。目前针对CFD的单克隆抗体药物APL-2已完成二期临床试验,通过抑制补体旁路途径显著降低地理萎缩患者的病灶扩大速度。CFD抑制剂还被探索用于阵发性睡眠性血红蛋白尿症和IgA肾病的治疗,已有小分子化合物进入临床前研究阶段。
